Escaped bugs result in one small change to a standard, not a new ceremony.

One person is not the only reviewer / releaser / firefighter.
Every critical area has a backup who has actually touched it recently.
A new teammate could find the playbook and ship something in week one.
Hygiene
We killed or simplified at least one rule in the last quarter, or looked and nothing deserved to die.
